The CamAPS FX app establishes a connection with continuous glucose sensors, such as the Dexcom G6 or FreeStyle Libre 3 transmitter, employing low energy Bluetooth for data transfer. This data is processed to guide insulin administration through an insulin pump in a glucose-responsive manner, embodying the principles of hybrid closed-loop or automated insulin delivery.

One notable feature of the CamAPS FX app is its ability to send SMS alerts generated by the glucose sensor to parents and guardians. Additionally, alerts can be monitored via the Companion mode of the app, which facilitates remote tracking of glucose levels—key safety features for caregivers.

The application also offers cloud data upload capabilities, enhancing data visualization for better management of diabetes.

Modes of Operation

The CamAPS FX app operates in one of two modes:

1. Auto mode Off (open loop)
   This mode is familiar to current pump users where the pump adheres to a pre-programmed basal profile or user instructions. Auto mode Off is the default setting upon system start-up.
2. Auto mode On (closed loop)
   In this mode, insulin delivery is dynamically directed by the app, superseding any pre-programmed basal insulin delivery. If conditions prevent activation of Auto mode—such as unavailability of CGM data—the app will continue attempting to engage Auto mode until resolved. During this 'attempting' phase, insulin delivery will revert to the pre-programmed basal rate after approximately 30 minutes.

SMS-based Remote Monitoring

The CamAPS FX app supports SMS-based remote monitoring in both Auto mode On and Off. Alerts generated by the app can be transmitted via SMS to up to five designated 'Followers.'

How Closed-loop Works

The CamAPS FX app utilizes a mathematical model to regulate insulin infusion, targeting a glucose level around 6 mmol/L. For optimal functioning, setup and operational information are required. Body weight serves as a parameter for estimating glucose and insulin concentrations, while the total daily insulin dose offers initial insights into insulin sensitivity, refined through analyzing CGM data, prior insulin infusions, boluses, and meal intake.

Data regarding previous insulin infusions and boluses help update individual characteristics such as insulin sensitivity. The mathematical model uses these refined metrics alongside active insulin and meal information to predict future glucose levels and determine the appropriate insulin infusion rate needed to maintain target glucose levels. In certain scenarios—like rapidly decreasing or low CGM glucose—the control algorithm may further reduce insulin delivery to mitigate hypoglycemia risk.
